The confluence of the overall economy, the real estate market in Phoenix, and the changes put into play by our contracted insurance companies have made the long-term viability of the business unprofitable, and therefore impossible.
Our last day of business operations will be January 31st, 2012.
In order to ensure the continuity of your care, we have been in discussions with a nearby physical therapy clinic that we are very comfortable referring to you. This clinic, Ideal Rehabilitation, has multiple convenient locations and also focuses on manual physical therapy.

We are very sorry to have to bring Northcutt Physical Therapy to its end. However, we have no regrets about the positive environment we created, and the talented staff we have been privileged to work with. Why is the clinic closing?
The state of the economy has impacted almost everyone. The response of the insurance companies to the changes put in place under the Affordable Healthcare Act and the resulting modifications to provider contracts have made the long-term viability of the business unprofitable. In order to survive as a single location private practice entity, the changes that would have to be made to the operations and level of care we currently offer are not acceptable to us.
What do I do about transferring my prescription and records to a different clinic?
Before your last treatment, ask the front office to print your most recent prescription for your current course of care so you can take it with you. You can also request your latest Progress Note. Your new clinic will ask you to fill out all of the demographic information and intake paperwork required for their practice and the therapist will perform an Initial Evaluation which will start your medical record with that facility.
Your complete patient record can be made available to you for a charge of $40.00 and will be available after February 15th by contacting: northcuttptmedicalrecords@gmail.com.
What if I have an insurance authorization on file? Does the authorization transfer?
Unfortunately there is no standardized methodology in healthcare for authorizations. Each case will be different. Very likely your authorization for treatment will transfer to the new location but you will want to ask that location to call to re-verify your authorization. Ask our front office to print out for you the latest authorization information we have on file so you can take it with you.
What/Who is Ideal Rehabilitation?
Ideal Rehabilitation is a very successful multi-location physical therapy facility. Ideal emphasizes manual physical therapy and has a holistic approach concerned with identifying and maximizing quality of life and movement potential within the spheres of promotion, prevention, treatment/intervention, and rehabilitation.

Will all my information and appointments transfer over to Ideal if I decided to go there?
To start treatment at any new clinic, you will be required to complete new patient intake forms which include all of your basic demographic information and a medical history. Our front office has a supply of New Patient Packets from Ideal for your convenience; or you can print it from their website: www.idealrehab.com.
None of the appointments you have on record with Northcutt PT will be valid after January 31st. You should contact Ideal immediately to make your therapy appointments for February.
Will my new physical therapy clinic be contracted with my insurance company? Will I have the same co-pay or co-insurance?
You need to check before you make an appointment to ensure that the physical therapy clinic you choose is contracted with your insurance plan. We always encourage you to call your insurance company to understand what your coverage may be with that particular healthcare provider. Will my treatment plan change?
Your treatment plan is very specific to you and is developed in conjunction with your Physical Therapist with your input. If you continue to see your current PT, your plan will very likely remain the same depending on the current progression of your care.
What is going to happen to the staff at Northcutt Physical Therapy?

What about Sheila Northcutt?
Sheila is taking this opportunity to re-locate to California, which is a first step in her extended retirement plan. In the short term, she will continue to see patients at a clinic in California and will further pursue options for teaching and seeing private clients.
