Your First Visit. Please read this letter before your first visit. It explains what to expect, what to bring, how to dress, etc.
Conditions of Admissions. This general release form covers assignment of benefits, acknowledgement of privacy statement, etc. Please print, read carefully and sign before your first visit.
HIPAA Patient Privacy. This is the privacy statement referred to in the Conditions of Admissions form, please read this before you sign that form.
Medical History. Please print and fill out this form before your first visit.
